Mind You by Lorraine Walker is fully committed to protecting the privacy and security of your personal records held.
This privacy notice describes how we collect and use personal information.
I am responsible for the holding of personal information about you and I am required under the data protection legislation to notify you of the information recorded in this privacy notice.
In the context of my therapy practice, why do I collect data?
Your personal data is recorded on clinical forms, these are required to be completed at the start of therapy treatment.
The information I collect includes
Your name, address, email, telephone number & any medical history.
Details on why you are accessing therapy and all other related information that is relevant to your successful therapeutic outcomes.
I hold your details in a personal clinical file which is stored securely on password protected electronic files.
Any hard copies are stored in a lockable filing cabinet in a locked office.
There will be updates to this notice as and when required and will review it annually in line with other policies and procedures.
I will of course ensure that you are notified of any updates to this privacy notice.
We will comply with data protection law. This says that the personal information we hold about you must be:
1. Used lawfully, fairly and in a transparent way.
2. Collected only for valid purposes that we have clearly explained and not used in any way without your consent.
3. Relevant to the intended purpose.
4. Accurate and kept up to date.
5. Kept only as long as our retention periods identified in our Retention of Records Policy.
6. Kept securely identified in our Data Protection & Confidentiality Policy

This policy forms part of our Privacy Notice to ensure compliance under General Data Protection Regulation (Regulation (EU) 2016/679 (GDPR).
Mind You by Lorraine Walker a division of Walker & Walker Logistics Ltd is required under certain operational and legislative guidance to collect, process, and securely store records relating to its clients and the businesses under its consultancy, business support and clinical therapy practice.
We are also required by law to retain information for a certain period of time.
We will be retaining our records for the default time period of 7 years.
